To schedule an office visit please call (916) 661-5336. In most cases, we can accommodate same day appointments. Co-payment and deductibles are due at the time of the office visit. Timely payment of bills is appreciated. We accept credit cards (VISA and Master Card) checks or cash. Returned check fee is $30.00.
NO SHOW FEE is $30.00.
It is my policy NOT TO PRESCRIBE ANTIBIOTICS or NEW prescription for pain medication without an evaluation. Infrequent exceptions (and as an example) is for bladder infections.
I prefer to discuss health concerns or answer any questions over the phone. I usually return calls around noon and after 4 pm on the same business day unless I am waiting for a final laboratory, imaging, or consult report in which case I will keep you up to date on the status within the next three days. Although there is a portal available (after you have established with our office as a patient), the best and soonest way to address your inquiry is still by phone. It is only my staff Tasha and I who answer the phone when we are not attending to other patients. Please leave a message and the nature of your concern so we can address it promptly.
Forms
A $20 fee will be assessed on any forms that you need filled out by Dr. Lu (e.g. DMV, EDD, IHSS, FMLA).
If you wish to have printed copies of medical records released to you, a cost will be assessed depending on the bulk of your records.
Medication Refills
For those who take medications on a daily or continuous basis, please check your medication bottle first if there are refills left in which case please ask your pharmacy for a refill. If there are no refills left, you are more than welcome to leave a message at our office with your name, your call back number, medication you are requesting, and the pharmacy name AND location. Since the new guidelines for CHRONIC CONTROLLED SUBSTANCES came out in October of 2019, we no longer prescribe such prescriptions from this office.
I usually give enough refills for daily medications taken for a chronic condition (e.g high-blood pressure) until your next due appointment. If you are on your last bottle of refill, it is time to come in for a follow-up. You may also be due for a blood test prior to your office visit.
If I have not seen you for a year, medication refill requests will be denied. If you are requesting a NEW medication I have never prescribed for you, I will need to evaluate you first face to face. For both cases, please call our office for an appointment. Feel free to leave a message and we will return your call usually on the same business day.
